You only need to complete this form once. This acts as a supplementary form for each of the participating schools.

Before you start you will need:

  • Your child's full details including name, address, date of birth. If your child will be in Year 6 in September 2021 and their date of birth is outside of the range 01/09/2010 to 31/08/2011 inclusive, please call 020 8642 3821 for further advice before commencing your registration.
  • A good quality recent (within the last year) digital photo clearly showing your child's face ready to upload (less than 2MB or 2048KB ) in size. See Photograph Guidance
  • Name, address (including postcode) and headteacher of your child's current primary school.
  • The name of the local authority to which you pay your council tax.
  • Your full details, including email address

The email address you give will be the only one used by the schools for communication purposes regarding the entrance tests (including any second stage tests) so it should be one you check regularly and will continue to use throughout the process.
